Conditions of Entry

1. All entries must be lodged electronically.
2. All entries must include one colour head and shoulder photo in jpeg format at high resolution.
3. The Entrant must agree to being entered.
4. Past winners cannot be entered in the award category they were successful/won previously.

6. A current curriculum vitae must be provided.
7. An overview of the entrant’s demonstrated excellence in the nominated Award category (no more than 1000 words). This may include significant achievements, leadership, commitment to personal/vocational/career development, future goals.
8. An overview of the entrant’s demonstrated contribution and impact on the Western Australia, National or International Community (no more than 1000 words). This may include level of impact and reach, length of contribution, who benefits, social, economic, cultural/other.
9. Celebrate WA reserves the right to publish entrants’ photos and biographical details.
10. Celebrate WA reserves the right to vet and substantiate information contained in the entry forms.
11. Celebrate WA reserves the right to have the entry considered in an alternative Award category if deemed appropriate.
12. All entry materials remain the property of Celebrate WA.
13. No responsibility can be accepted for lost or misplaced entries and incomplete entries will not be considered.
